he State of Connecticut Department of Mental Health and Addiction Services (DMHAS) is seeking a dedicated and compassionate Community Clinician to join the Assertive Community Treatment (ACT) Team at River Valley Services in Middletown, CT. This position plays a vital role in delivering intensive, community-based services to adults with significant behavioral health and substance use conditions who require frequent, hands-on support to maintain stability and successfully live in the community.

As a Community Clinician, you will provide trauma-informed, recovery-oriented care through clinical interventions, case management, crisis response, and likage to essential resources. You'll work collaboratively with a multidisciplinary team to ensure clients receive consistent outreach and individualized support aligned with the ACT model.

Provide clinical assessment, case management, and individualized treatment planning for an ongoing caseload.
Deliver crisis intervention and risk management services in community settings.
Assist clients in linking to community supports, medical care, housing, and entitlements.
Monitor medication compliance and coordinate care with psychiatrists and nurses.
Facilitate education and training in daily living skills and community integration.
Maintain accurate, timely documentation in accordance with ACT model and accreditation standards.

River Valley Services (RVS) is a community mental health center serving towns across Middlesex County. We provide high-quality crisis intervention, case management, residential, and clinical services to adults with serious mental health conditions who may not have access to private care. Our programs include short-term residential respite, community support, mobile crisis response, jail diversion, and other essential services that promote recovery and independence.

ABOUT THE ASSERTIVE COMMUNITY TREATMENT TEAM:
The Assertive Community Treatment (ACT) team provides intensive, community-based services to individuals with serious behavioral health and substance use challenges who often struggle to engage in traditional treatment. ACT is an evidence-based model that uses a multidisciplinary team approach to deliver highly responsive, individualized, recovery-focused care in clients’ homes and communities.

Qualifications & Requirements
MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE
A Bachelor’s degree in a health care field and two (2) years of experience working independently using clinical case management techniques in a behavioral health setting.

NOTE: Case management is defined as primary responsibility for the implementation, monitoring, evaluation and assessment of client progress towards meeting treatment objectives as outlined in an approved treatment plan.

MINIMUM QUALIFICATIONS - SUBSTITUTIONS ALLOWED

A Master’s degree in counseling, psychology or other clinical discipline may be substituted for one (1) year of the General Experience.
A current Connecticut license as a Registered Nurse OR a Multistate or Compact Registered Nurse license and two (2) years of experience working in a psychiatric setting may be substituted for the Bachelor’s degree.
A Master’s degree in social work (MSW) and two (2) years at the level of a Mental Health Assistant 2 independently performing the full range of paraprofessional case management in a behavioral health setting may be substituted for the General Experience.

PREFERRED QUALIFICATIONS
Experience working in community setting providing case management services such as linking to community resources, medical care, entitlements with people with complex behavioral health and substance use related conditions
Experience working with adults with complex behavioral health and substance use conditions
Experience working within a multi-disciplinary team
Experience with assessing risk and crisis intervention
